APPOINTMENT REQUEST FORM Please fill out the form below to submit a request to be tattooed by Luke Kelly, aka @thebabaluke, at Glitch Tattoo. After submitting, you will receive an email in less than a week letting you know if your request has been accepted or denied. If accepted, your response email will include a link to pay your deposit and select your appointment date. Location on body * IE: Inner forearm, full chest, full back, side of calf, etc. Sizing * IE: half sleeve, quarter sleeve, hand or palm sized, 6x6 inches, etc. Other information Please add any additional information that you'd like. Is this piece in memoriam? Have you been tattooed by Luke before? etc. You may list any of this here if so, and if not, tell us your favorite video game, movie, or tv show! DISCLAIMER As listed on our home page, Luke operates off of a flat day rate. This means you are the only appointment for the entire day and pricing is the same each day. If your idea is smaller, please feel free to submit multiple ideas in the same request. Luke can typically tattoo one large design or 2-3 palm sized designs in a day and if your idea is accepted, we will always let you know if your piece will require multiple sessions. Luke's current flat day rate is $1200, with a $200 deposit that is taken off the final price of your tattoo and due when you select your appointment date. THERE IS ABSOLUTELY NO COST TO SUBMIT THIS FORM. Thank you for submitting your request to be tattooed by Luke at Glitch Tattoo! While in most cases we try to respond in 24-48 hours, in rare cases it may take up to week for a response. If your request is accepted, you will be emailed a link to pay your deposit and select your appointment date.
